Denial code lookup: what the code means and how to fix it

What CO, PR, OA and PI mean

COContractual obligation. The provider absorbs it; do not bill the patient.
PRPatient responsibility. Bill the patient (deductible, coinsurance, copay, non-covered).
OAOther adjustment. Neither group applies, often coordination of benefits.
PIPayer-initiated reduction. The payer adjusted it; check the contract.
